In order to request that your medical records be sent to an outside facility, please download and complete the medical records release form below, and then send to the address or fax number listed at the bottom.

Your medical record request will be completed within
3-5 business days, and we will inform you if we experience an unexpected delay. Please note that after the request is completed, delivery to your address may vary due to postal delivery times.
Additional Information
For Radiology films, please contact the Facility or Hospital where your imaging was done.
For billing requests or billing records from Premier Surgeons only, please call 719-576-4171.
For all other billing requests or billing records related to your surgery, hospital stay, or any additional Providers that may have treated you, please contact their billing office directly. Questions about your hospital/surgicenter bill should be directed to the hospital/surgicenter billing department.
